Subject Description

    This subject examines changing demographic and epidemiological patterns and underlying determinants of health in order for students to analyse changing primary health care service roles. Through developing a deep understanding of the health service population students will be able to effectively plan, manage and evaluate primary health care programs to meet population health needs. Opportunities to integrate evidence-based disease prevention and health promotion activity in routine practice will be explored.

Learning Outcomes

  • critically analyse the characteristics of a health service population including the underlying determinants that impact on population health outcomes
  • analyse and design strategies to enhance prevention and early detection of disease
  • investigate, analyse and evaluate methods of adding population health value to individual care and program activity in health care settings
  • design health program planning and evaluation processes in health care settings
  • critically reflect on the challenges of reforming workplace activity and analyse approaches to manage change effectively
