This subject will examine the complexity of chronic illness, disability, rehabilitation and palliative care. It will focus on the development of a holistic approach to nursing care to assist the novice nurse to provide safe and compassionate care to those who endure chronic illness and/or disability, and require rehabilitation or palliative care.
Learning Outcomes
- integrate knowledge of the pathophysiological and psychopathological changes occurring with individuals living with a range of chronic health conditions and/or terminal illness;
- appropriately assess the client with a chronic illness or disability;
- discuss nursing strategies used in the management of individuals experiencing a range of chronic health problems and those with terminal illness;
- apply rehabilitation principles to prevent complications; maintain and restore function for those individuals experiencing chronic health conditions and disabilities;
- describe and apply supportive responses to individuals and families experiencing chronic health conditions and those living with terminal illness through the provision of increasingly complex therapeutic nursing management;
- recognise the need for a seamless transition of care between settings.
