Enrolment in this subject is restricted to all students enrolled in the Bachelor of Physiotherapy, Bachelor of Physiotherapy IHCAP and the Bachelor of Physiotherapy Honours Programs.

This subject aims to develop professional knowledge, clinical reasoning and problem solving skills within a 'discharge planning' or 'case management' framework. This will enable students to design optimal discharge plans for clients with chronic and complex needs. The subject makes use of targeted lectures and practical sessions. In addition, case studies will be used to provide students with the opportunity to apply clinical reasoning, problem solving and an evidence based approach to the preparation of discharge plans for people with complex needs. Students will work both as an individual and within a group in the preparation of comprehensive resource folders for use in clinical practice.

Learning Outcomes

• upon successful completion of this subject, students should be able to apply the ICF framework to identify critical factors that need to be addressed prior to discharge and in the long term management of people with chronic or complex conditions;
• upon successful completion of this subject, students should be able to explore the range of resources available to support people with chronic or complex conditions, determine the appropriate resource required and identify the process for accessing those resources;
• upon successful completion of this subject, students should be able to design a management plan for a 'case' with a chronic or complex condition that is evidence-based, appropriate to an individual's context and inclusive of their future needs;
• upon successful completion of this subject, students should be able to present group written, oral and visual information around a 'case';
• upon successful completion of this subject, students should be able to construct a 'Resource Folder' for use in clinical practice that includes interventions, services and funding for management of complex cases and that is appropriate to the context of Queensland.
